Why Can’t I Lose Weight? The Science Nobody Explains

This might be one of the most searched health questions there is, and also one of the most frustrating to live through. You are doing everything you think you are supposed to do, and the number will not move. I have been there, and what I found underneath it was not a lack of discipline. It was cortisol.

The Stress Hormone Nobody Blames

Cortisol is your body’s primary stress hormone, and it plays a much bigger role in weight than most people realize. When your body perceives ongoing stress, whether from insufficient sleep, chronic dieting, or simply too much physical or emotional load, cortisol rises. Elevated cortisol over time promotes fat storage, particularly visceral fat around the midsection, and can work directly against weight loss efforts (Anderson et al., 1987, Journal of Clinical Endocrinology & Metabolism).

This matters because most weight loss advice assumes the only variables are food and exercise. Stress is a third variable that can override both.

Overtraining and Undereating Make It Worse

This is the part almost nobody talks about, and I learned it the hard way. Training hard while not eating enough does not accelerate fat loss. It compounds the same problem.

Intense exercise is itself a physical stressor. Combine that with insufficient caloric intake, and cortisol rises further, pushing your body into a defensive, fat-conserving state rather than a fat-burning one (Hackney, 2006, Expert Review of Endocrinology & Metabolism). Chronically elevated cortisol has also been linked to increased cravings, disrupted sleep, and a slower metabolic rate over time, the exact opposite of what most people are trying to achieve when they push harder.

The uncomfortable truth is that under-fueling while overtraining can stall weight loss just as effectively as eating too much.

The Hormonal Piece Nobody Mentions

For women specifically, hormonal shifts change this equation significantly, and perimenopause is one of the biggest examples. As estrogen fluctuates and eventually declines, insulin sensitivity tends to decrease, and fat distribution shifts toward the abdomen, even without any change in diet or exercise (Davis et al., 2012, Journal of Clinical Endocrinology & Metabolism). This is a physiological shift, not a personal failure, and it means that what worked for weight management in your twenties or thirties may genuinely stop working the same way later.

What Tests Are Actually Worth Asking For

If weight loss has stalled despite consistent effort, these are worth discussing with a doctor:

• Cortisol (morning blood or saliva test) to check for chronically elevated stress hormones

• Thyroid panel (TSH, free T3, free T4), since thyroid function directly affects metabolic rate

• Fasting insulin and glucose, to assess insulin resistance

• Estrogen, progesterone, and FSH, especially relevant for perimenopause

• Vitamin D, which plays a role in both metabolic and hormonal function

The Actual Cheat Sheet

Chronic stress raises cortisol, and elevated cortisol promotes fat storage, especially around the midsection

Overtraining combined with undereating raises cortisol further, not less

Perimenopause changes insulin sensitivity and fat distribution independent of diet or exercise

If progress has stalled despite consistent effort, hormone and metabolic testing is a reasonable next step, not a last resort

Weight loss is not purely a math equation of calories in and out. Stress and hormones are real, measurable variables

The Takeaway

Weight loss that stalls is not usually a sign you are not trying hard enough. More often, it is a sign your body is under more stress than your effort accounts for. Sometimes the answer is not pushing harder. It’s addressing what’s actually happening underneath.
